Lumbosacral Radiculopathy: An In-depth Guide to Diagnosis and Management

Low back pain stands as a ubiquitous musculoskeletal complaint, representing a significant challenge in clinical settings. Its widespread occurrence and substantial impact underscore the importance of considering the extensive range of differential diagnoses, notably lumbosacral radiculopathy. This condition is a primary driver of disability in the developed world, particularly for individuals under 45. Lumbosacral radiculopathy is characterized by pain stemming from the compression or irritation of nerve roots in the lumbosacral spine, often accompanied by numbness, weakness, and altered reflexes. Critically, it can manifest without pronounced lumbar pain, making accurate and timely diagnosis essential for effective intervention. The condition frequently arises from degenerative processes such as disc herniation, ligamentum flavum thickening, facet joint hypertrophy, or spondylolisthesis, all of which can lead to nerve root compression.

Diagnosis hinges on a comprehensive physical examination, including provocative tests like the straight leg raise test (Lasègue test), and may necessitate magnetic resonance imaging (MRI) if symptoms persist despite conservative treatments. These initial treatments often include nonsteroidal anti-inflammatory drugs (NSAIDs), acetaminophen, and gabapentin. Successful management typically requires a collaborative interprofessional healthcare team approach to optimize patient outcomes. Introduction

Low back pain is a leading musculoskeletal complaint encountered in clinical practice. In the developed world, low back pain is a major cause of disability for individuals aged 45 and under, second only to the common cold as a cause of missed workdays.[1] The economic burden of low back pain on healthcare systems is substantial annually. While epidemiological studies vary, the incidence of low back pain is estimated to be greater than 5%, with lifetime prevalence rates ranging from 60% to 90%.[2] Fortunately, many instances of low back pain are self-limiting and resolve without medical intervention. Approximately half of all cases resolve within 1 to 2 weeks, and 90% resolve within 6 to 12 weeks. Given the diverse range of potential underlying causes of low back pain, it is crucial to include lumbosacral radiculopathy in the differential diagnosis.

Lumbosacral radiculopathy is a pain syndrome resulting from the compression or irritation of nerve roots in the lumbosacral region of the spine. This nerve root compression is frequently caused by degenerative changes such as disc herniation, alterations in the ligamentum flavum, facet hypertrophy, and spondylolisthesis. These conditions can lead to the compression of one or more lumbosacral nerve roots. The characteristic symptom is low back pain that radiates into the lower extremities in a dermatomal pattern, corresponding to the specific nerve root affected. Additional symptoms can include numbness, muscle weakness, and changes in reflexes; however, the absence of these symptoms does not rule out lumbosacral radiculopathy.[3]

Etiology of Lumbosacral Radiculopathy

The underlying cause of lumbosacral radiculopathy is the noxious stimulation of a spinal nerve root. This stimulation leads to the generation of ectopic nerve signals, which are perceived as pain, numbness, and tingling sensations along the nerve’s distribution. In more severe cases, weakness can also occur in the muscle groups (myotome) innervated by the affected nerve root. Lesions of the intervertebral discs and degenerative spinal disease are the most common causes of lumbosacral radiculopathy. In the context of degeneration, intervertebral discs undergo desiccation and fibrosis, followed by fissuring or tearing. This process increases the risk of disc herniation, where disc material protrudes beyond the normal boundaries of the disc space. These herniations can directly compress one or more nerve roots, leading to lumbosacral radiculopathy. In extreme cases, a large disc herniation compressing multiple nerve roots can result in cauda equina syndrome, a serious condition requiring immediate medical attention.[4] Beyond degenerative causes, any process that irritates the spinal nerve roots can potentially lead to lumbosacral radiculopathy.

Epidemiology of Lumbosacral Radiculopathy

The lifetime prevalence of low back pain within the general population is remarkably high, ranging from 60% to 90%. Of these individuals, 5% to 10% will develop radiculopathy.[2, 5] Lumbosacral radiculopathy is a significant reason for referrals to specialists such as neurologists, neurosurgeons, and orthopedic spine surgeons. Low back pain, including radiculopathy, is the second most common cause of workplace absenteeism, following only upper respiratory tract infections. Approximately 25 million people in the workforce miss work each year due to low back pain, and over 5 million are disabled as a result. Patients with chronic back pain account for a considerable portion of healthcare spending.[3] Established risk factors for the progression of lumbar degenerative changes, which can lead to radiculopathy, include smoking, genetic predisposition, and repetitive physical trauma.[6]

Pathophysiology of Radiculopathy

Lumbosacral radiculopathy is a clinical term describing a predictable pattern of symptoms that arise from mechanical or inflammatory processes affecting one or more lumbosacral nerve roots. Patients may experience a range of symptoms, including radiating pain, numbness, tingling, weakness, and gait abnormalities, varying in severity. The specific symptoms and their distribution depend on which nerve root is compromised. Symptoms typically follow predictable patterns affecting the corresponding dermatome (area of skin supplied by a single nerve) or myotome (muscle group supplied by a single nerve). [7]

In degenerative disc disease, the intervertebral disc undergoes desiccation and fibrosis, which can lead to fissuring or tearing of the annulus fibrosus. This weakening increases the likelihood of disc material herniating beyond the normal disc space. The posterior longitudinal ligament, which is strongest in the midline, and the posterolateral annulus fibrosus may bear a disproportionate amount of weight from the upper body. This anatomical stress distribution may explain why the majority of lumbosacral disc herniations occur posterolaterally within the central canal or lateral recess. Herniated discs in these locations can compress nerve roots, resulting in lumbosacral radiculopathy. In the lumbosacral region, herniated discs typically compress the traversing nerve root as it enters the lateral recess before exiting through the neural foramen at the level below. For example, an L5-S1 disc herniation will most commonly compress the S1 nerve root.

History and Physical Examination for Radiculopathy

As with any medical condition, a thorough patient history and physical examination are critical for the accurate diagnosis of lumbosacral radiculopathy. Pain is the most frequently reported symptom. However, patients may also describe numbness or weakness that follows the dermatomal or myotomal distribution of the affected nerve root. Radicular pain is often characterized as “electrical shocks” or “shooting pains” that radiate along the dermatome of the nerve root. During history taking, it is essential to screen for any red-flag symptoms that might indicate a more serious or emergent underlying condition.

Before proceeding with a detailed evaluation, clinicians must first rule out any associated “red-flag” symptoms. These symptoms could suggest a more severe underlying pathology requiring immediate medical attention. Red-flag symptoms include: thoracic pain, fever or unexplained weight loss, intravenous drug use, immunosuppression, prolonged steroid use, night sweats, bowel or bladder dysfunction, and a history of malignancy. Additionally, documentation should include any previous surgeries, chemotherapy or radiation treatments, recent imaging studies, bloodwork results, or a history of metastatic disease. These factors may be associated with pain at night, pain at rest, unexplained weight loss, or night sweats. Significant medical comorbidities, neurological deficits or worsening neurological exam findings, gait ataxia, saddle anesthesia, acute onset of urinary retention or overflow incontinence, progressive lower extremity weakness, and age of onset (either younger than 20 or older than 55) are also important considerations.

A comprehensive neurological examination should be performed, including assessment for upper motor neuron signs such as the Babinski sign, clonus, and spasticity. During the physical examination, several maneuvers can assist clinicians in diagnosing lumbosacral radiculopathy. The straight leg raise test (Lasègue test) is performed by passively raising the patient’s leg while they are supine. This maneuver increases tension on the sciatic nerve, typically eliciting pain between 30° and 60° of leg elevation from the examination table in patients with radiculopathy. The crossed straight leg raise test, while less sensitive, is more specific for radiculopathy. A positive test is indicated if the patient’s radicular symptoms are reproduced during passive leg elevation between 30° and 60°, suggesting involvement of the lower lumbar nerve roots (L4 to S1). Conversely, the reverse straight leg raise test (Ely test) stretches the femoral nerve and the L2 to L4 nerve roots. This is performed with the patient in a prone position by extending the hip and flexing the knee. The slump test is another provocative maneuver where the patient is seated with the neck fully flexed and knees fully extended to provoke radicular symptoms.[8, 9]

In the context of lumbosacral radiculopathy, diminished deep tendon reflexes are often observed. Specifically, the knee-jerk reflex may be reduced or absent in L4 nerve root compression, the medial hamstring reflex may be affected with L5 nerve root involvement, and the Achilles reflex is commonly diminished or absent with S1 nerve root compression. Motor weakness associated with lumbosacral radiculopathy typically affects specific muscle groups depending on the nerve root involved: the quadriceps femoris (knee extension) for the L4 nerve root, the tibialis anterior and extensor hallucis longus (dorsiflexion of the foot and big toe) for the L5 nerve root, and the gastrocnemius (plantarflexion of the foot) for the S1 nerve root. Sensory deficits are also common, with decreased sensation noted along specific dermatomes: the medial malleolus and medial aspect of the foot for the L4 nerve root, the dorsum of the foot for the L5 nerve root, and the lateral malleolus and lateral aspect of the foot for the S1 nerve root.

Evaluation and Diagnostic Testing for Radiculopathy

Given that the majority of low back pain symptoms and lumbosacral radiculopathy cases have a favorable prognosis and often resolve spontaneously, extensive imaging is generally not necessary for patients with symptoms of less than 4 to 6 weeks duration. The initial diagnostic approach should prioritize a comprehensive physical examination. However, the presence of neurological deficits, particularly focal muscle weakness identified during the physical exam, necessitates further investigation. If symptoms persist beyond 1 to 2 months despite conservative management, magnetic resonance imaging (MRI) without contrast is considered the gold standard for evaluating lumbosacral radiculopathy. In cases where infection is a concern, MRI with contrast is recommended, especially for patients with a history of prior spinal surgeries (see Figure. Lumbar MRI).

A computed tomography (CT) myelogram is an alternative imaging modality for patients who are contraindicated for MRI. However, CT is less sensitive than MRI in visualizing soft tissues and tumors and is not routinely recommended for radiculopathy diagnosis unless MRI is not feasible. Plain radiographs (x-rays) are readily available and simple to perform. They can reveal gross bony abnormalities such as fractures, spondylolisthesis, disc space narrowing, and other degenerative changes. In situations where radiographic findings are inconsistent with the patient’s clinical presentation, electromyography (EMG) and nerve conduction studies (NCS) can be helpful to localize a lesion with relatively high diagnostic specificity.[10]

Treatment and Management Strategies for Radiculopathy

Treatment approaches for lumbosacral radiculopathy vary depending on the underlying cause and the severity of the patient’s symptoms. Conservative management is typically the first-line treatment strategy. Medications are used to manage pain, including nonsteroidal anti-inflammatory drugs (NSAIDs), acetaminophen, anti-epileptic drugs that are effective for nerve-related pain (e.g., gabapentin), and in cases of severe pain, low-dose opioid analgesics. Systemic corticosteroids are also an option for managing lumbosacral radiculopathy and can provide modest benefits for pain relief.[11]

Non-pharmacological interventions are frequently employed in the management of radiculopathy. Physical therapy, acupuncture, chiropractic manipulation, and traction are commonly utilized. However, it is important to note that the evidence supporting the efficacy of these treatment modalities is not definitive and remains somewhat equivocal. Interventional pain management techniques are also commonly used, including epidural steroid injections and percutaneous disc decompression procedures. In cases where conservative and interventional treatments fail to provide adequate relief, surgical decompression may be considered. Instrumented spinal fusion may also be performed in conjunction with decompression if there is concern for spinal instability.[12, 13, 14]

Differential Diagnosis of Lumbosacral Radiculopathy

The differential diagnosis for lumbosacral radiculopathy is broad and includes a variety of conditions that can mimic its symptoms. Key differential diagnoses include:

  • Degenerative Conditions of the Spine (most common causes):

    • Spondylolisthesis: In degenerative spondylolisthesis, a cascade of events involving intervertebral disc degeneration, subsequent intersegmental instability, and facet joint arthropathy leads to vertebral slippage.
    • Spinal Stenosis: Narrowing of the spinal canal can compress nerve roots, causing radicular symptoms.
    • Adult Isthmic Spondylolisthesis: While typically caused by a defect in the pars interarticularis, the degenerative subtype is more prevalent in adults. Pars defects (spondylolysis) in adults are often secondary to repetitive microtrauma.
  • Trauma (burst fractures with bony fragment retropulsion):

    • Spinal fractures, especially burst fractures with retropulsion of bone fragments into the spinal canal, can directly compress nerve roots. Clinicians should be aware that spinal fractures occur in both younger individuals due to high-velocity injuries (e.g., motor vehicle accidents, falls) and in older individuals, particularly those with osteoporosis, from low-velocity injuries or even spontaneously. Associated bleeding from traumatic injuries can lead to rapid neurological deterioration.
  • Benign or Malignant Tumors: Spinal tumors, both metastatic and primary, can cause radiculopathy by compressing nerve roots. Metastatic tumors are more common, but primary tumors such as ependymoma, schwannoma, neurofibroma, lymphoma, lipomas, paraganglioma, ganglioneuroma, osteoblastoma, and multiple myeloma should also be considered.

  • Vascular Conditions: Certain vascular conditions can mimic radiculopathy, including hemangioblastoma, vascular claudication, and arteriovenous malformations.[16]

  • Peripheral Conditions: Conditions affecting the peripheral nerves, such as peripheral neuropathy and piriformis syndrome, can present with pain radiating into the leg, similar to radiculopathy.[18]

  • Congenital Conditions: Congenital anomalies, such as conjoined nerve roots, can predispose individuals to radiculopathy symptoms.

Prognosis of Lumbosacral Radiculopathy

The prognosis for lumbosacral radiculopathy is generally favorable. The majority of cases resolve with conservative management over time. For patients who require surgical intervention, studies indicate good outcomes. A study following 100 patients who underwent discectomy for radiculopathy reported that 73% experienced complete relief of leg pain, and 63% had complete relief of back pain at the 1-year follow-up.[21] These positive outcomes were maintained at the 5- to 10-year follow-up, with 62% reporting sustained complete pain relief in both leg and back pain categories. Notably, only 5% of patients were classified as having failed back surgery syndrome at the 5- to 10-year follow-up.[22]

Potential Complications of Radiculopathy Treatment

Complications associated with surgical intervention for lumbosacral radiculopathy are relatively uncommon but can occur. These include superficial wound infection (reported in 1% to 5% of cases),[23] increased motor deficit, unintended incidental durotomy (tear of the dura mater), recurrent herniated lumbar disc (approximately 4% recurrence rate at 10-year follow-up),[24] and postoperative urinary retention.

Lumbosacral radiculopathy is a pain syndrome caused by compression or irritation of nerve roots in the lumbosacral spine. Common symptoms include low back pain, leg pain (sciatica), numbness or tingling in the lower extremities, and muscle weakness. Often resulting from degenerative changes in the spine, lumbosacral radiculopathy can frequently be managed effectively with conservative treatments. These treatments include medications, non-pharmacological approaches such as physical therapy, and localized pain management techniques like targeted injections.

If symptoms do not improve with conservative management, further evaluation with radiographic imaging and, in some cases, surgical intervention may be necessary. Individuals experiencing symptoms suggestive of lumbosacral radiculopathy should seek medical evaluation to rule out red-flag symptoms and receive appropriate diagnosis and management. Prompt assessment and treatment are crucial for conditions associated with these symptoms.

Pearls and Key Considerations in Radiculopathy Management

Lumbosacral radiculopathy is a common condition in clinical practice, contributing to a significant number of annual physician visits. While most cases are benign and resolve spontaneously, conservative management is the recommended initial approach for patients without red-flag symptoms. However, if symptoms persist despite conservative measures, or if neurological weakness or red-flag symptoms are present, diagnostic studies such as radiographic imaging, electromyography, and nerve conduction studies are valuable tools for confirming the diagnosis and guiding further management.[16, 7, 14]

Enhancing Healthcare Team Outcomes in Radiculopathy Care

Lumbosacral radiculopathy is a prevalent condition, and it is essential for all members of the healthcare team, including advanced practice providers and physicians, to collaborate effectively. Vigilance in recognizing red-flag symptoms that require emergent interventions is paramount. Timely communication with the physician and coordinated team-based management are critical for optimizing patient outcomes in the management of lumbosacral radiculopathy.
